Medically Home, A DispatchHealth Company, operates a decentralized care model for patients with serious, complex or high-risk illnesses.

We work together with our health system partners to provide safe, hospital-level care for patients at home. To do this, we use a comprehensive platform that includes our own clinical protocols and reimbursement model. We help run the platform, and our partners help too, including managing patient referrals and other services. This care model is preferred by both patients and healthcare providers, and it leads to better clinical and financial results compared to traditional care methods.

In June 2025, Medically Home merged with DispatchHealth, bringing together two industry leaders to form one of the nation’s most comprehensive and established healthcare platforms focused on delivering complex care at home.
